ALL Award Entry Forms and the State Statistical Form are available via the Member Portal under Membership on the RESOURCES Tab.

Download the needed forms. DO NOT COMPLETE FORMS ON SMART PHONE OR TABLET!

If you have questions on your Award Entry, please reach out to the Chairman.

Once complete, save to your computer system and submit to the GFWC-NC 1st Vice President.

Annual Club Report: GFWC-NC State Statistical Form (SSF): Please log into the Member Portal to access the GFWC-NC State Statistical Form.  This document is completed by Club Presidents annually & is due to the GFWC-NC 1st Vice President by February 1st.
